New Patient/Medical History Forms
Thank you for considering my office as your new Dental Home. I bring 32 years of family care to Pasadena. I would describe our office as Old School One on One care with a 21 st century flair! Your initial visit will be with Dr. Lucas and will entail a Complete Exam and appropriate radiographs as determined by Dr. Lucas. Please allow up to 1 hour for this visit. If you have access to current radiographs, please have them available for this first visit. If you have been up to date with your dental visits with a previous dentist, you will have your dental prophylaxis done on your first visit. If it has been 3 years or more, be prepared to return for a second visit.
